Parents of Anwatin students are required to complete the following forms and return them to school. These forms are available at school or can be downloaded at the bottom of this page.

Information about the Citywide Discipline and Internet Use policies can also be found below. Parents do not need to print these forms. These items will be reviewed with students and signed in class.


Application for Educational Benefits (formerly known as the Free and Reduced Lunch Form) - required
All parents are asked to complete the Application for Educational Benefits. The information gathered in the application has an impact on our school budget. Please complete the application by December 1 to ensure that your child's school gets the funding it needs for the following programs:

  • Bilingual and bicultural programs
  • Family Engagement
  • CPEO – Connecting Parents to Educational Opportunities
  • Professional development
  • Direct Instructional Services
  • Remedial Instruction Services: Reading, language arts, mathematics, study skills
  • Additional teachers and teacher aides
  • Lower instructor-to-learner ratios
  • Instructional materials, digital learning
  • Truancy reduction programs

Household applications were sent out in the mail on August 14th, 2017. To request another paper application, please contact the school's main office at 612.668.2450. If you prefer, you can do the application online through your SchoolCafe account at https://www.schoolcafe.com.  It is important to know that participation in public schools, including the free/reduced price lunch program is available to all children regardless of their immigration status or their parent's status. Please contact the school if you have any questions. Thanks!

2017-2018 Parent Assessment Refusal Form - optional

Parents may refuse to allow their children to take State and District assessments by submitting this completed form to their school’s Test Coordinator. To ensure that your child’s school is able to fulfill this request, please submit your completed form by the return date listed in the document. Every effort will be made to honor requests submitted after the dates listed, but the deadline allows time for the Test Coordinator to notify building staff as well as make other arrangements for your child during testing.
